Is an RN escort needed to transport a stroke patient who has received TNK?

Question#: 608

Question:

Question came up today after a stroke meeting. Is an RN escort needed to transport a stroke patient who has received TNK? Normally RNs come as escorts with patient receiving tPA, but that is for the ongoing infusion requirements. Interested in hearing your thoughts.

Thanks.

Answer:

A TNK bolus may be considered as any other medication bolus given in ED prior to interfacility transfer. Therefore, paramedics may conduct interfacility transfers of stroke patients who have received a bolus of TNK without a nurse escort.
