Pregnant Paramedics & Methoxyflurane Administration

I remember from the in person training provided in 2024 the facilitators mentioning that pregnant Paramedics we’re not to administer Methoxy nor supervise pt’s self administration of that drug.
I just performed the online review and no mention is made in that regard.
We currently have a few pregnant Paramedics on the road in our service and I believe it would be pertinent to clarify this prior to Methoxy re-introduction which is coming up in May.

Answer:

During the initial study phase, pregnant paramedics were advised not to administer methoxyflurane or supervise patient self‑administration. This guidance was precautionary in nature and reflected the limited occupational exposure data available at that time. Since then, additional high‑quality evidence has emerged.

A controlled laboratory study published in 2025 examined occupational exposure to methoxyflurane in a simulated ambulance setting, including worst‑case scenarios involving repeated administrations during a shift. The study demonstrated that methoxyflurane exposure levels remained well below established 8‑hour time‑weighted average and short‑term exposure limits under all conditions tested. Proper ambulance ventilation reduced exposure by over 95%, and no adverse health effects were observed among participants. These findings support that environmental exposure during supervised self‑administration is minimal when standard ventilation and correct use of the activated carbon filter are followed.

It is important to note that while pregnancy remains a contraindication for patients receiving methoxyflurane, occupational exposure for paramedics is fundamentally different from direct patient administration. However, no studies have specifically examined pregnant paramedics administering or supervising methoxyflurane, and data in this specific population remains limited.

Given this, it would be reasonable that:

  • Administration or supervision of methoxyflurane by pregnant paramedics be at their discretion
  • If they choose to participate, ensuring enabled ambulance ventilation is recommended
  • Paramedics who prefer not to administer methoxyflurane can opt out without issue and use other medications for pain management

Based on the available evidence, administration or supervision of methoxyflurane by pregnant paramedics should remain a matter of individual paramedic discretion, guided by personal comfort and perceived safety.
