SIRS/Sepsis parameters
Question# 921
It was recently brought to my attention that within the OBGH app under Medical References, the sepsis notification tool in the RPPEO region does not follow the evidence-based criteria for SIRS (for HR and RR). Is there a rationale behind this?
Answer:
Hello and thank you for your MedicASK question.
You are correct in noting that the medical reference in the OBHG App is not updated to current guidelines. The most up-to-date parameters are:
SIRS Criteria
SIRS (Systemic Inflammatory Response Syndrome) is defined as two or more of the following that are new to the patient.

SIRS Criteria
SIRS (Systemic Inflammatory Response Syndrome) is defined as two or more of the following that are new to the patient.
- Hyperthermia >38.3°C or Hypothermia <36°C
- Acutely Altered Mental Status
- Tachycardia >90 bpm
- Tachypnea >20 bpm
- Hyperglycemia (6.7mmol/L) in the absence of diabetes
- Leukocytosis (>12,000 μL-1) or Leukopenia (<4,000 μL-1) (out of scope)
Sepsis Criteria
Two or more SIRS signs that are new to the patient
PLUS suspicion or diagnosis of new infection
The OBHG app currently was showing the old criteria that was used to guide paramedics in early sepsis recognition and more accurate CTAS grading.
We have reached out to those that manage the OBHG app and the updated criteria is now located under a new heading of SIRS/Sepsis.
