This two-day virtual workshop (Sept 11 & 12) will provide Paramedics with background theory and hands-on practice in incident analysis using Root Cause Analysis (RCA) and in proactive risk assessment using Failure Mode and Effects Analysis (FMEA).
Day 1:
Incident Analysis – Root Cause Analysis (RCA)
The workshop curriculum is derived from the Canadian Incident Analysis Framework.
Program Abstract:
We’re all human – errors happen. In our field, when errors happen, they can have traumatic consequences. In this hands-on workshop, we will analyze adverse events in a way that takes the blame away from the individual and evaluates them using a systematic approach. “What was the expected outcome and why did that not happen?” On Day 1, participants will learn how to conduct a Root Cause Analysis and we’ll use diagramming to deconstruct and understand how adverse events occur.
Day 2:
Proactive Risk Assessment Using Failure Mode and Effects Analysis (FMEA)
The workshop curriculum is derived from the Canadian Failure Mode and Effects Analysis Framework Version II (2016).
Program Abstract:
Day 2 builds on the principles learned on Day 1, with a change in focus to proactive risk assessment and process redesign. What do we know about system failures and how can we identify problems before they occur? Through interactive activities, participants will use a technique called Failure Mode and Effects Analysis (FMEA) to design processes that take human factors into account and decrease the likelihood of a failure impacting a patient.
Audience: Paramedics, physicians, risk managers, patient safety officers, and other health care professionals.
Workshop Level: Introductory

The Regional Paramedic Program for Eastern Ontario (RPPEO) is excited to announce 2 newly added Trauma Days to the Fall CME lineup.
Following successful Spring 2025 Trauma Days, Eric Gagnon is back to deliver his popular and informative Trauma Day Continuing Medical Education (CME) sessions.
*NEW* - Level 1 Trauma Day & Level 2 Trauma Day
We recognize that there is a varying level of training across paramedics in our region. With this, we want to provide you with the opportunity to attend a day that best suits your learning needs and wants so that you can get the most out of the day. Below is a breakdown of the differences between the different levels of Trauma Day. All paramedics can sign up for either level, but we encourage you to look at and follow our recommendations:
Level 1
· Recommended for those who have not attended a previous Trauma Day
· New entry-to-practice paramedics (<5 years recommended)
· Higher focus and time spent on skills review and practice
· Less focus on scenarios
Level 2
· Recommended for those who have attended previous Trauma Days
· Experienced paramedics (5+ years recommended)
· Higher focus and time spent on bringing a variety of skills together
· Higher focus on scenarios with high fidelity simulations
Continued Hybrid Format for Greater Accessibility
To make the training accessible to more learners, Trauma Days will continue to be offered in a hybrid format. This includes:
