Does the RPPEO require paramedics to complete IRs for any type of calls?
There is a belief they are required an IR for all VSA, is this correct? Or is an IR required only if the patient is not transported?
I have looked through your policies and cannot find any reference to my question above as well as Medic Ask, i don't see any reference.

Answer:
The RPPEO does not mandate paramedics to complete IRs. This requirement is set by the Ontario Ambulance Documentation Standards. Currently, we follow Version 3.0 (effective April 1, 2017), though a draft of Version 4.0 is available (https://www.ontario.ca/page/ontario-ambulance-documentation-standards#section-3)
For details on incident reporting requirements, refer to Section 2 of Version 3.0.
Additionally, individual ambulance service providers may have their own documentation expectations for paramedics, but these fall outside the RPPEO’s scope.
That said, the RPPEO encourages paramedics to report patient safety incidents through our Patient Safety Incident Reporting (PSIR) form (https://www.rppeo.ca/forms-menu2/patient-safety-incident-report-form).
Guided by Just Culture principles, we review cases where paramedics raise concerns about ** near misses, patient safety incidents, medical errors, or adverse events. This system allows us to identify and analyze your concerns, collaborate on solutions, continuously improve patient safety, and facilitate systems changes before an incident occurs.
