As a PCP, we are compelled to transport if a shock has been administered by FD or paramedic crew (medical TOR contraindicated) . If the family on scene expresses a verbal DNR, is it reasonable to patch to the BHP to ask for a TOR, respecting the patient's/family's wishes?
Generally we “always assume viability”” and transport whenever we can. The ALS PCS is written with this in mind. In special circumstances, it is always permissible to patch to the BHP for guidance and permission for something within your scope of practice. Keep the call moving towards transport realizing that the answer may be “no” or there may be a patch failure.
